When you complete a self-referral, the information you provide helps us ensure that you receive the most appropriate care and support tailored to your needs. We use your data to:

  • Assess your referral and determine the services that may be right for you.
  • Communicate with you about your referral and the care we can provide.
  • Coordinate with healthcare professionals, if needed, to ensure the best outcomes for your care.
